I don't think I've ever heard of it happening in the case of a sloppy, disjointed, story or a draft with a rushed ending.

If it's a fic with "[ending will go here]" instead of an ending, or chunks of lorem ipsum to meet wordcount, or something like that, then it may very well be forcibly defaulted, but there have been cases in which even obvious placeholders made it to the collection opening before being sent out as emergency PHs. IIRC one story was just the recipient's letter copied and pasted over and over.
